The Registered Nurse is a licensed professional who provides direct and indirect professional nursing care to patients/clients under a physician’s plan of care and in accordance with the policies and procedures of the Agency, compliance with the conditions set forth in the State Nurse Practice Act, and regulatory entities.
Director of Nursing and/or Director of Patient Services
Licensed Practical Nurses/Licensed Vocational Nurses, Home Health Aids, Certified Nursing Assistants, Caregivers
Education and Experience:
- Currently licensed as a Registered Nurse to practice in the State of Arizona.
- Have a minimum of one (1) year of clinical experience as a professional nurse within the past three years or completion of a refresher course in the last year.
- Has an ADN/BSN from an accredited program by the National League of Nursing and/or has a Public Health Nursing certificate, if required by state.
- CPR certification, current.
- Valid Arizona driver’s license and proof of current Automobile insurance.
- Valid State of Arizona Department of Public Safety Fingerprint Clearance Card.
Duties and Responsibilities:
- Follow Sunrise Health and Hospice “21 Points of Light and Mission”.
- Follows Agency Policies and Procedures, Federal and State Regulations and Standards of Practice.
- Attends Agency In-services, especially annual fire, safety, HIPAA, Universal Precautions, and Blood borne pathogens.
- Attends and participates in Monthly Multidisciplinary Case Conference providing information concerning patients/clients and input into care and discharge planning.
- Follows Agency procedure of obtaining patients through intake scheduling coordinator.
- When accepting patient, agrees to make skilled visits to assigned patients/clients as scheduled.
- Notifies patient/clients prior to visit and communicates further visits scheduled.
- Drives responsibly and safely to and from client’s home.
- Schedules own time based on priorities and maintains productivity utilizing time management.
- Provides appropriate and thorough service to patients/clients, ensuring proper completion of services in an accurate and timely manner and in accordance agency policy.
- Makes the initial patient/client Start of Care (SOC) assessment within forty eight (48) hours from time referral was received and as scheduled by the agency.
- Demonstrates respectable customer service and professionalism at all times.
- Assess patient’s/client’s overall health status, systems review and safety environment to include fall risk assessment, hospitalization risk assessment and Braden assessment.
- Establishes Home Bound Status and document.
- Provides skilled services and/or supervises, requiring substantial knowledge and skills in accordance with the Plan of Care (POC) and accepted standards of professional practice.
- Determines discipline frequencies according to physician order and/or assessed needs.
- Implement and adhere to physician’s orders where such orders are within the legal scope of professional nursing practice, performing venipuncture and/or obtaining specimen for lab and transport to appropriate testing site as ordered.
- Obtains physician order or verbal orders for Start of Care (SOC) or Resumption of Care (ROC) or Recertification (RECERT) within twenty four (24) to forty eight (48) hours from time of service.
- Notify physician with any changes with the initial plan of care (POC) and obtain any appropriate orders.
- Provide teaching to patient/client and answer health-related questions and document patient/client/caregiver understanding of education.
- Complete Home Health Nursing Aide care plan and make supervisory visits on Home Health Aide’s every fourteen days (14).
- Perform Supervisory visit every six (6) months for LPN and document.
- Document patient’s/client’s care using appropriate agency forms and submit to agency within seventy two (72) hours from time of visit.
- Assess any medication changes with each Re-visit and update Medication Profile Form in the home and in the agency’s chart.
- Notifies physician with any change in client condition, abnormal findings and new medications. Obtains orders as indicated with the appropriate form and submit in a timely manner not to exceed seventy two (72) hours from time order was obtained.
- Completes Discharge Oasis and Discharge Summary when patient/client is discharged.
- Complete Recertification Oasis every sixty (60) days while on service and obtain new orders for new plan of care.
- Communicate with staffing coordinator with any scheduling concerns and/or changes in patient/client assignment.
- Turn weekly schedule to agency.
- Communicate all Start of Cares (SOC), resumption of care (ROC) or recertification (RECERT) to the clinical support division within twenty four (24) hours of completion.
- Maintain continuity of care and document communications with others within the health care team.
- Demonstrate work that meets quality standards. Assist in periodic auditing of paperwork of self/peers.
- Assist in training of new personnel for field duty. Assist in training student nurses.
- Communicates regularly with Director of Nursing and/or Director of Patient Services with all patient/client concerns or ideas for better patient/client outcomes.
Quality Improvement/Professional Development/Customer Service
- Adheres to Agency Code of Conduct, Standards of Practice, and Patients/Clients Rights/ Responsibilities.
- Demonstrates professionalism in appearance, adhering to Agency dress code.
- Demonstrates professionalism in behavior and attitude while in patient’s homes, at Agency, communicating to staff and when orienting new hires.
- Comply with Universal Standard Precautions/ Occupational Exposure to blood borne pathogen and/or toxic substances.
- Demonstrates compliance with HIPAA.
- Demonstrate compliance with Oasis forms and Knowledge of Home Health regulations.
- Accessible transportation to and from patients/clients visits.
- Ability to be adaptable, flexible and able to maintain effectiveness during change.
- Ability to effectively handle difficult people and/or occasional high stress environments.
- Skilled in interpersonal relationships, verbal and written communication skills.
- Possesses ability to demonstrate the nursing process/assess, plan, implement and evaluate the patient’s/client’s needs.
- Ability to communicate information concerning a patient’s/client’s condition clearly to physician, other field staff and Director of Nursing.
- Ability to understand and carry out directions or instructions and keep records.
- Possesses leadership and supervisory ability.
- Knowledgeable of nursing and medical practices, procedures, regulations and guidelines that pertain to home health care.
- Knowledge of the theory, methods, and techniques required for rendering patient care.
- Assumes delegated authority and accountability for total care of assigned patients/clients.
- Functions independently with minimum supervision.
- Understand and use proper body mechanics to prevent injury to self and/or patients/clients.
- Ability to lift 30 pounds, move intermittently throughout the work day, inclusive of sitting, standing and bending , pushing, pulling or moving objects and patients/clients.
- Communicate with Director of Nursing and/or Director of Patient Services with any patient/client procedure questions, concerns, grievances and/or compliments.
- May provide Director of Nursing and/or Director of Patient Services assistance to updating current Policies and Procedures and/or better ways of processes to improve the quality of patient/client care.
Continuing Education Requirements
Licensed personnel are expected to participate in appropriate continuing education as required by state regulating boards. In addition, licensed personnel are expected to accept personal responsibility for other educational activities to enhance job related skills and abilities.
